Registration costs include a boxed lunch. You will choose from the six options described below during the registration process. If you register THE DAY OF THE CONFERENCE, you will not have all these options; rather, we will order a variety of extras that you can choose from.

Cancellations and Refunds
A handling fee of $10 will be assessed on all refund requests received by 5:00 p.m. Thursday, September 1, 2011. No refunds will be available after this date. Mail requests to Utah County Hospice Coalition Workshop Refunds, 169 Harman Continuing Education Building, Provo, UT 84602.

Admission and Standards
Participants of any race, color, creed, and ethnic or national origin are accepted for admission to this program provided they maintain the university standards while on campus. These include modesty in dress and conduct and abstinence from alcohol, tobacco, coffee, tea, and harmful drugs.
