Note: Registration is limited. We encourage you to register early.

• Regular: September 1 – 7, 2011
• On Site: September 8, 2011

Registration costs include a boxed lunch. You will choose from the six options described below during the registration process. If you register THE DAY OF THE CONFERENCE, you will not have all these options; rather, we will order a variety of extras that you can choose from.

• Turkey Breast Sandwich Box: Freshly sliced turkey served on wheat bread with lettuce and tomato. Potato chips, mustard and mayonnaise packets. BYU Chocolate Chip Cookie. Deli-style pickle wedge.
• Ham & Cheese Sandwich Box: Fresh ham and cheese on wheat bread with lettuce and tomato. Potato chips, mustard and mayonnaise packets. BYU Chocolate Chip Cookie. Deli-style pickle wedge.
• Roast Beef Sandwich Box: Served on a hoagie bun with lettuce and tomato. Potato chips, mustard and mayonnaise packets. BYU Chocolate Chip Cookie. Deli-style pickle wedge.
• Garden Vegetable Sandwich Box: Fresh lettuce, pico and guacamole, with cucumbers, peppers, cheese and balsamic vinaigrette on ciabatta. Comes with potato chips, fresh fruit and Brigham Creek Salsa.
• Spinach & Veggie Wrap Box: Garden-fresh spinach, guacamole and pico, sprouts, mushroom and cheese with Brigham Creek Salsa. Comes with fresh fruit and potato chips.
• Turkey Wrap Box: Spinach tortilla wrap with turkey breast, cream cheese, tomatoes, cucumber, olives and fresh romaine. Served with BYU Ranch Dressing, a side of fresh fruit, and potato chips.

Cancellations and Refunds
A handling fee of $10 will be assessed on all refund requests received by 5:00 p.m. Thursday, September 1, 2011. No refunds will be available after this date. Mail requests to Utah County Hospice Coalition Workshop Refunds, 169 Harman Continuing Education Building, Provo, UT 84602.
