I-9 Form Instructions

This is a required US Government document.

EFFECTIVE FEBRUARY 1, 2012: You will need to provide a copy of any document used for the i9. If you come into the EFY Office, we will make a copy of the verifying document for you. But if you are mailing in the form, we will need you to provide a copy of the document used. Please note, this does not mean that you can bring a copy of your document when having the i9 filled out. You are still required to show the EFY/CES person the original document as this is what makes you legally eligible to work.

If you are able to come into the EFY office in person, please

  • Bring your original documents with you (e.g., passport, Social Security card, Driver License, etc. -See the I-9 attachment below for all document options).
  • Do not complete an I-9 Form prior to coming in to the office. As BYU now participates in eVerify, you will be completing the form online. (E-Verify is an Internet-based system that allows businesses to determine the eligibility of their employees to work in the United States http://www.uscis.gov/portal/site/uscis.)

If you are not able to come into the EFY office, please

  • Take the I-9 Form and your original documents (e.g., passport, Social Security card, Driver License, etc. – see the I-9 attachment below for all document options) to the nearest CES full-time employee (e.g., institute director, instructor, secretary/staff or a seminary teacher). Documentation filled out by volunteers cannot not be accepted.
  • Refer to the sample I-9 below for guidance on how to fill this form out. It is your responsibility to fill out only Section 1 completely, including signing, dating and marking the appropriate boxes. If you neglect signing and dating this form, it will be invalidated, so please do not miss this very important detail. Section 2 can only be filled out by a full-time CES employee who is seeing your original documentation. Please leave section 3 blank.

CES EMPLOYEE Guide for Section 2:
Please be sure to fill out list A if the applicant brings in their passport. Otherwise leave A blank and fill out both B and C. DO NOT fill out all three as this will invalidate the form.

Listed below is the link to print out the I-9 Form. A second link “Sample I-9 Form” shows three examples for completing the form with the most commonly used documentation. Please use this link to ensure that the form is correctly filled out the first time.

International Students/Instructors may not need to fill out an I-9 if they are not working at a session in the United States. Before filling out this document, please contact (801) 422-3817, ext. 2 to see if this is a necessary document.
