EFFECTIVE FEBRUARY 1, 2012: You will need to provide a copy of any document used for the i9. If you come into the EFY Office, we will make a copy of the verifying document for you. But if you are mailing in the form, we will need you to provide a copy of the document used. Please note, this does not mean that you can bring a copy of your document when having the i9 filled out. You are still required to show the EFY/CES person the original document as this is what makes you legally eligible to work.

Please be sure to fill out list A if the applicant brings in their passport. Otherwise leave A blank and fill out both B and C. DO NOT fill out all three as this will invalidate the form.
Listed below is the link to print out the I-9 Form. A second link “Sample I-9 Form” shows three examples for completing the form with the most commonly used documentation. Please use this link to ensure that the form is correctly filled out the first time.
